A patient started a new prescription that she felt made her frequently irritable, to the point of “not feeling like herself”.

The patient found she was having trouble going to work and keeping her appointments with her provider. In response, her Care Coordinator, Jermerne Nicholas, scheduled an appointment for TV medication management to find the source of the problem and promised to follow-up with the patient after.

A week later, the patient reported to Jermerne that she had started a new prescription that made her feel “better and brighter” in mood and able to go to work and appointments without issue. At the end of the follow-up, the patient told Jermerne that she really appreciated Jermerne’s care in quickly addressing the source of her concerns and following up as promised.
