Scheduling with a Dietitian
In addition to care coordination, your care team includes access to a licensed dietitian as part of your care management program.
A dietitian can support your care plan with:
- Condition-specific nutrition recommendations to improve your health.
- Focused adjustments for dietary restrictions like lactose intolerance, gluten intolerance, and allergies.
- Practical advice on meeting any of your physician’s suggested dietary changes.
- Easy-to-understand explanations of the connection between nutrition and your health.
Even if you feel confident about your daily nutrition, a dietician is a great team member to have. Think of them as an extra tool in your toolkit for getting the most out of your care plan.

These calls with the dietitian are offered as part of the Chronic Care Management (CCM) or Principal Care Management (PCM) service which you are already enrolled for with your provider.
These calls are scheduled separately from your normal care coordinator to allow a focused conversation on diet and nutrition.
