Cosan Care Alerts
Cosan supports Care Management services for physicians and medical providers including programs such as Chronic Care Managment, Behavioral Health, and Remote Patient Monitoring.
When you consent to a Cosan supported Care Management service supplied by your medical provider, you will receive SMS (“TXT”) alerts. You may have enrolled with a form in your doctor’s office, as part of as part of a visit with your physician, or during a recorded conversation with a Cosan Care Coordinator.
If you recently provided a new phone number, you will receive request from our short code number. Reply “YES” to opt-in.
At any time, you can ask your Care Coordinator to update your phone number for these alerts, or opt out during your call. Alternatively, you can reply “STOP” at any time to discontinue TXT notifications.
These alerts include reminders about your upcoming call with your Care Coordinator and notifications if we missed your call.
For patients who have remote patient monitoring devices (such as a blood pressure cuff device) from their doctor, you will also receive alerts if your device has been offline.

Message frequency varies – You should expect 1 to 8 SMS/TXT notifications per month as determined by the number of Care Management scheduled calls you require. Remote Patient Monitoring patients may receive up to 1 additional SMS/TXT message each day a device is offline, up to 31 notifications per month.
